Consent and Waiver of Responsibility
I/We the parents/legal guardians of the above-named participant on a Lakeside Lutheran Jr. Warrior Football and Badger East YFL Team, hereby give my/our approval to his/her participation in any and all league activities during the current season. I/We do hereby waive, release, absolve, indemnify and agree to hold harmless the Badger East YFB League, its officers and organizers, the individual Clubs/School and their coaches and participants, their School Districts, Sponsors, Supervisors, and any and all persons involved in transporting my/our child, except to the extent and in the amount covered by Accident or Liability Insurance.

Financial responsibility for athletic uniform(s) and equipment
As parent (or legal guardian) of the above named student-athlete, I agree to be financially responsible for the prompt and proper return of all athletic equipment issued to him/her. I understand that my son is responsible for any uniform/equipment that is assigned to them and agree to pay the replacement value of the uniform/equipment in the event that it is lost, stolen or damaged. (Except for normal wear and tear)
Assumption of risk
I/We realize that there are risks in athletic activities provided by Lakeside Lutheran High School and that my son may incur serious injury, even death, as a result of his/her participation in such athletic activities. I have weighed these considerations and approve of the participation of my son named on this page.
Insurance
I certify that I have adequate insurance coverage on my son, the above named student-athlete, to cover expenses in the event of an athletic related accident or injury.
Consent for medical treatment
This consent extends the right to arrange for immediate medical treatment by a licensed physician and/or other trained medical personnel, and for them to provide such emergency medical care as deemed appropriate to preserve the life or well being of my child. I hereby release, hold harmless, and indemnify Lakeside Lutheran High School and the Badger East YFL, its officers and organizers, the individual Clubs/Schools and their coaches and participants, their School Districts, Sponsors and Supervisors from any injury or damage related to administration of emergency medical care as authorized herein. This consent for Medical Treatment is in effect for the duration of the 2025 football season.
