This form is only to be used for Lakeside groups, teams, etc. that need to reserve a specific portion of the school for a meeting, team meal, end of year party, etc. This form MUST be filled out in order to reserve the Auxiliary Kitchen.

This form is NOT to be used for outside groups looking to rent facilities at Lakeside, those requests go directly to the Activities Director, Todd Jahns.
