Participation Forms

All students participating in athletics are required to complete and turn in the following forms before gaining eligibility:

  1. WIAA Physical Form

    • Incoming Freshman and Juniors typically need this form
    • Needs to be signed and dated by a doctor

  2. WIAA Alternate Year Physical Form

    • Sophomores and Seniors typically need this form
    • Needs to be filled out completely and signed by a parent/guardian

  3. 2017-2018 Participation Consent Form

    • Needed by all students participating in athletics
    • Please read the “WIAA Eligibility Information Bulletin” and the “LLHS Student-Athlete Handbook” (below) before signing this form

  4. Parent & Athlete Concussion Agreement

    • Needed by all students participating in athletics
    • Please read the “Concussion Information for Parents” and the “Concussion Information for Athletes” (below) before signing
    • A signature is needed by both a parent/guardian and the athlete

  5. ImPACT letter Consent Form

    • It is needed by all new students to Lakeside participating in athletics.
    • Please read the “ImPACT” parent letter (below) before signing this form
    • A signature is needed by both a parent/guardian and the athlete

All necessary forms should be completed and returned together to Lakeside Lutheran no later than August 1. Forms may be mailed or dropped off in the front office. View/Download forms by clicking on the form name above or stop in the front office of school to request them. Make your appointment for a physical as soon as possible after April 1.
 

Athletes will not be able to try out or practice for a team without the required forms on file.
